• Individual members: $75 per year is the recommended amount, or pay what you can ($20 minimum).

• You can add a second member ($150 total for both recommended, $40 for both minimum), but they each will need to provide a unique email address.

• Life members: In appreciation of their dedication to democracy, Life Members (members for more than 50 years) are exempt from membership dues.

• Dues distributions: 20% of dues will go to the local League (of your choice), 47% will go to the state League, and 33% will go to LWVUS.

• The preferred payment option is a direct bank transfer (ACH transfer). You’ll need your financial institution’s routing number and your account number.

• You also have the option to pay by credit or debit card.

If you prefer to pay by check:

• Please make your dues check payable to: LWVUS.

• Print and complete this membership form. (Please include the name and email address of any additional household member included in the dues amount.)

• Mail your check and membership form to LWV Ohio, 471 E. Broad St. Suite # 1630, Columbus, OH 43215.

• You will receive an online confirmation and an email receipt for your payment.
